Nursing Diagnosis: Readiness for Enhanced Knowledge (Ackley, 2020)

Definition:  A pattern of cognitive information related to a specific topic or its acquisition, which can be strengthened.

Desired Outcomes:    

  • Client will: Understand risk factors for heart disease.
  • Client will:  Describe how to modify behavior and how to embrace a lifestyle that supports health.
  • Client will:  Determine personal lifestyle-modification goals.
  • Client will:  Meet personal lifestyle-modification goals.

Interventions: 

  • Establish rapport and a trusting relationship with client.
  • Use a guiding or facilitating style when working with clients who desire greater knowledge about health. Ask guiding questions to facilitate conversations and to foster critical thinking.
  • Use health coaching and motivational interviewing to help clients focus on lifestyle modification priorities and goals.
  • Refer client to lifestyle modification and health promotion resources in community or online.
